Counselor Dual Enrollment Upload Forms

All counselors or school personnel responsible for uploading dual student enrollment applications and corresponding documentation are required to create a Dynamic Forms account.

2. Click on the direct link https://dynamicforms.ngwebsolutions.com to create your new Dynamic Forms account. Please retain your Dynamic Forms login credentials for future use.
3. Once your account is created, select the proper information upload form below. Note: In order to successfully submit the forms to WCCS, all information is required to be entered and uploaded into this form.
